” I happen to be an ex-UH houseman and doing my obligatory posting in a small(er)-town hospital. I have to admit housemen from teaching hospitals lack the know-how of doing procedures — there is a lack of opportunity of doing so — you have to be pro-active in tertiary hospitals (and beg the MOs and compete with the other HOs!) if you want to be well-versed in these.

However in teaching hospitals, they emphasize on current guidelines and protocols on managing certain cases and in turn, the HOs are also updated on the current guidelines. We also get to see a lot of “hi-fi” cases (as I call it) or “weird and wonderful” cases (as my prof back in uni call it).

In small(er) hospitals, there is a lot of bread-and-butter cases surely. I enjoy the fact that I play an active part in managing patients along with my senior MO. Of course, I get to do a lot of procedures here. However it can be hectic and specialists sometimes do not have time to teach — as opposed to teaching hospitals that do have compulsory teaching sessions for housemen. It takes your own effort to update your knowledge yourself.

Whichever hospital you get for housemanship doesn’t really matter for me IMHO your attitude at work and the knowledge you glean from your experience matter more, doesn’t it? “

    I am graduating in a month time. My family lives in kajang, and i still have a hard time of choosing which hospital to go for housemanship. I am not familiar with malaysia medical system, i havent lived in the country for such a long time. I have interest in internal medicine, i love cardiology if that helps you advicing me.
    I got some questions for you, up there you chose kajang hospital as your first choice. I heard somewhere the wind told me that if i did my housemanship in kajang, which is selangor, it will be hard for me to get a position in let say, selangor and kuala lumpur for MO. Now is this true?
    Second, from my own experience during my internship in a tertiary hospital, i had more time touching medical books rather than a patient. And when i was sent to a district hospital for 6months and the opposite happened, i attended new patients and old patients and this and that but the teaching were not that good in district hospital. Which hospital that has good teaching power and offers hands-on patient that much?
    Last but not least, did you get kajang hospital for your HO? How was it?

    • Hi there drmfmd, my personal experience is that,

      1. I did my housemanship in Hosp Serdang for 2 years.
      2. Then I continued my MO in Hosp Kuantan for 2 years
      3. Now I have relocated myself in Hosp Kajang as a medical MO

      In terms of housemanship, I have enjoyed doing my housemanship in Hosp Serdang. I have enjoyed my days in Medical department in Hosp Kuantan, and I am learning that Hosp Kajang is also nice.

      I think you have to go through all rotations of housemanship before you can say I love cardiology. i did my cardiology rotation in Hosp Kuantan, and yes it is an interesting postings.

      In terms of asking where to do housemen, find a big hospital first. So that, you will learn more, A big hospital has the facilities and most importantly senior consultants. Senior consultants and specialist will teach you more. Your MO are way too busy to teach, because they themselves are also studying. I managed to get my posting in Hosp Serdang because I guess it was a lucky option.

      Getting into Selangor as a medical officer is difficult already because the bulk of the doctors comes from here. But not too worry, because other states hospital also are in need of MOs. These states hospital are also not too far from Selangor.

      Interms of housemanship, now it is more leanient. You will have ample of time to study, if you are managing your time correctly. It doesn’t matter if no one willing to teach, as long as you keep the interest up, everyone is available.

      I guess that is all for now.

      • Okay thanks, that helps a lot. So it is possible to go back to the federal territory given that you paid back your housemanship time else where?

  3. “paid back housemanship time” is quite incorrect. Housemanship is a training period and MO-ship is a service period. It is a different thing. During Housemanship you should learn as much, because when you become an MO it is a different game.

    I am not sure whether there is a ruling as such.

    But try to enjoy every bit of struggle during working.
